Notification of Mental Health Commitment Form

Using GTS to generate the Notification of Mental Health Commitment form is not required. However, this feature is available for counties to use if they would like a more automated method of producing the form.

The person must have an adjudication of incapacity and an address recorded in GTS in order to produce the form. For more information, see Adjudication of Incapacity Case Action.

Generating the Notice of Mental Health Commitment Form

To generate the form, perform the following:

1. 

Under Find a Case on your Dashboard, enter the case number and press ENTER. Optionally, if the case number is not readily available, click the Advanced Search link to locate the case. The GTS Case screen opens.

2. 

Above the Case Actions grid on the Case Actions tab, click the Print MH Commitment icon Print MH Commitment icon. The Mental Health Commitment Form Data popup screen displays.

3. 

Record the additional information which will print on the form. Some fields may already be populated from the case details and participant record of the incapacitated person.

4. 

Click the SAVE button to return to the GTS Case screen.

5. 

A new case action for the Mental Health Commitment form displays in the grid and the form displays as an attached document. Click on the Document icon Document icon in the row for the Notification of Mental Health Commitment Generated case action.

6. 

The form opens in a new window or tab which can be used to print it. When finished, return to the tab in your browser for the UJS Web Portal to continue.

7. 

You may return to your Dashboard.
